Reach and Reconnect

Our Reach and Reconnect health and digital literacy project is funded by The National Lottery Community Fund and tackles health inequalities, supports pandemic recovery and living alongside Covid, and facing the cost-of-living challenges. The project responds to the needs of local communities served by the Consortium and its delivery partners and builds on, and enhances, existing health improvement work by addressing health and digital literacy and capacity gaps by specifically offering:

 

Health Literacy, targeted support to:

  • Better understand how to prevent ill health
  • Better manage existing conditions
  • Improve access to primary care and community pharmacies
  • Improve self-management skills and accessing local peer support
  • Instil confidence to support family members
  • Enable access to specialist services

 

Digital literacy targeted support to:

  • Equip residents with day-to-day digital skills, including safety and security, especially those with English as a second language.
  • Overcome digital barriers, enabling online access to:
  • Health and healthcare e.g. appointments, self-care plans, health apps, ordering prescriptions
  • Information, support, and advocacy relating to wider factors impacting on health, e.g., housing, skills and employability, loneliness and isolation.
